Real-world Medicare data (Zalawadiya 2025, n=9,579) show baseline PAD >20 mmHg is associated with ~32% higher 2-year mortality vs PAD ≤20 mmHg (adjusted HR 0.68 [95% CI 0.62–0.73] for the ≤20 group). Pooled remote PA pressure sensor trial data (Zile 2025, n=4,019) show the relationship is monotonic and nearly linear across PAD 15–35 mmHg — each 1 mmHg of baseline PAD adds ~3–4% mortality risk (HR 1.04 [1.03–1.05]). Patients who reach acceptable PAD (≤20 mmHg) within 90 days of remote PA pressure sensor implant have ~28% lower mortality vs persistently elevated (HR 0.72 [0.64–0.81]). A target ≥20 mmHg warrants explicit confirmation that this reflects current clinical intent for this patient (sensor calibration, palliative ceiling, documented physiology) rather than an unreviewed default.
